The information you provide helps me understand your child's educational needs.  Submitting this information does not commit you to use my advocacy services, nor does it mean that I have agreed to advocate on behalf of your child.  An initial consultation must accompany a complete, copy of your child’s records.  Please know, we have all written things in haste and had things we regret or see as mistakes documented … I promise I have my fair share.  It is very important that I have complete information in order to effectively advocate for your child.  I cannot return the information you send.  Please do not send originals.  You may not have all of the documents that are listed. 

Suggested Items to Send:

• School  paperwork (SST, 504 Plan and related documents, Eligibility Documents, IEP’s, Report Card/Progress Reports

• Standardized Tests (COGAT, ITBS, CRCT)

• All evaluations performed: Psychological/ Neuropsychological/ Educational Psychological, OT/PT/ST/AT/AP Evaluations

• Other pertinent information, educational documentation including letters and emails to/from the school

• Medical documentation from your private specialists regarding your child’s disability or special needs,

Please arrange all of the information you send in CHRONOLOGICAL ORDER, oldest document on top, most recent information on bottom.  If you must add dates or other information to the documents, use sticky notes.  Please do not write on, highlight or underline the documents themselves. 

Attach the New Client Information and History form to your email. 

I will email you back when I receive your records with a suggested day and time for our phone consult.  I will review and evaluate all information and we will discuss further assessments, steps, or assistance, which you may need to take.  We will discuss the vision you have for your child and ways that I can assist you.
